MEDICIAL INFORMATION
In order to care for your child in the best possible way, and for their own safety and wellbeing,
PARETNS DREAM require parents to provide full information on any relevant medical conditions,
allergies, additional needs and/or dietary restrictions.
You will be asked to provide the relevant information when registering your child to your online
account. Alternatively, when making bookings by telephone, our team will request this information
from you. We require you to check and confirm the information we hold for your child during the
bookings process. Any changes to circumstances to any information given then you must inform
the team straight away.
PARENTS DREAM reserve the right to exclude children for whom full information (as described
above) has not been provided, either from taking part in certain activities or (if felt necessary) from
attending
PARETNS DREAM clubs altogether until full information has been provided. It is your
responsibility to ensure we hold the correct information at all times and we reserve the right to
cancel a booking at any time where there has been a failure to provide said information. No refund
or credit will be issued in those circumstances.
6. LATE FEES
PARENTS DREAM reserve the right to charge a fee for late collection of any children. If a child is
collected after the allocated collection time identified on booking, the parent/guardian will be
subject to an immediate charge of £8.00 per child. An additional £8.00 will be charged for every
half hour which passes, up until collection. If the parent/guardian is unable to pay the late fee at
point of collection, it will automatically be added to the relevant account
7. SAFEGUARDING
PARENTS Dream staff have a duty to respond if they suspect a child in their care may be suffering

from abuse, or if a child makes a disclosure about abuse. In this event the relevant staff will follow
the Safeguarding Children’s Policy as detailed in our policies and procedures.
8. LIABALITY
PARENTS DREAM does not accept liability for personal injury or death of any participants unless
directly caused by the proven negligence of the company or its servants.
9. EXLUSION/SUSPENSION
PARENTS DREAM reserves the right to exclude/suspend or refuse a child without notice, if we
consider that their presence compromises the good atmosphere of the club. Transport home will
be the responsibility of the parent and no refund/credit will be issued.
10. PROGRAMME CHNAGES
PARETNS DREAM reserves the right to amend its services and activity programme, content,
times, dates and venue in the event of unsuitable weather conditions, failure of equipment,
building faults and any other operational faults that may arise from time to time, without refund or
compensation to the customer.
11. COMPLAINTS
If you or your child are not entirely satisfied with the service we have provided, we would like to
know about it. Any complaint should first be made to the Club Manager, who will complete a
complaints report, and do everything possible to ensure the complaint is dealt with as quickly and
effectively as possible. If the complainant feels that the outcome of the complaint is insufficient, or
would like to take the grievance further, the complaint should be made and emailed to

12. DATA PROTECTION
PARENTS DREAM committed to safeguarding your privacy; protecting the rights and freedoms of
data subjects and safely and securely processing their data in accordance with all our legal
obligation
13. ACKNOWLEDGMENT
I acknowledge that the participation in the activities provided by PARENTS DREAM entails
known and unknown risks that could result in physical or emotional injury, paralysis, death or
damage to themselves to property or to third parties. I further acknowledge that the activities
provided by PARENTS DREAM require a reasonable level of fitness and ability.
14. ACCEPTANCE
I hereby agree and promise to accept and assume all of the risks existing in this activity. My
participation in this activity is purely voluntary and I elect to participate in spite of the risks.
I expressly agree and promise to accept to be responsible for any and all risks existing and
subsisting in the activity from time to time and acknowledge that their participation in the
activity is voluntary and that they elect to participate despite of the risk and agree to accept
the consequences of so doing save only to the extent that Parents Dream except liability for
death or personal injury caused by the company’s negligence and / or breach of statutory
duty.
15. MINOR If the participant is a minor, I agree that this Release of Liability and Assumption of
Risk Agreement (“WAIVER / RELEASE”) is made on behalf of that minor participant and that
all of the releases, waivers and promises herein are binding on that minor participant. I
represent that I have full authority as Parent or Legal Guardian of the minor participant to bind
the minor participant to this Agreement.
16. UPDATE
In consideration of not being required to sign a fresh copy of this Waiver Release before
each visit, I further agree that this Waiver Release shall apply to all visits within the next 12
months by me and by the minor participant in full.
17. This Waiver/disclaimer will be governed by and construed in accordance with English law and
any disputes relating to this disclaimer shall be subject to the exclusive jurisdiction of the
courts of England.
18. Late FEES, Late fees will apply for every 15 minutes you are late to pick up. A £2.50 charge per 15 minutes you are late
