  • PAR requires crating or X-pens (that we provide) while you are away depending on age of dog. Are you willing to crate train them? *
  • I understand and agree to adhere to all relevant zoning and animal control codes and ordinances, whether local, country, or state.
  • Fostering a dog requires a large commitment of time and energy. Often while fostering a bond will develop between you and the foster dog that may make it very difficult to give up the dog to its new ‘forever home’. Have you considered this aspect of fostering, and are you prepared for this type of commitment and for letting go of the foster dog?*
  • ACKNOWLEDGE THE FOLLOWING:

  • There are inherent risks in fostering a dog. PAR shall not be liable for any damages, injury or death resulting from the inherent risks of fostering a dog. No person fostering a dog from PAR or person affiliated in any way with a person fostering a dog from PAR shall make any claim against PAR, or maintain any action against PAR, or recover from PAR for injury, loss, damages or death resulting from any of the inherent risks of dog fostering.

    By signing below, Applicant acknowledges the inherent risk in fostering a dog and agrees to waive any and all claims against PAR as set forth herein.

    By signing below, you attest to the truthfulness of the information that you have provided to PAR herein and otherwise.

    By signing below, you give PAR permission to contact your veterinarian and your references and visit your home prior to your approval for fostering. PAR is required to inspect each foster home at least once annually.

    By signing below, you agree that PAR may inspect your foster home annually if you are approved to be a PAR foster home.

    By signing below, you agree that if you provide any false or misleading information you may be disqualified from fostering and/or PAR may remove a fostered dog from your premises.

    By signing this application, I (we) testify that all of the above information is accurate and valid.
