Baptism Certificate
Please complete this form and upload a copy of your student’s Baptismal Certificate. If your student was baptized at Holy Family Catholic Church, you do not need to upload the Baptismal Certificate. Please note, your student must have also received the sacraments of First Communion and Reconciliation in order to receive the sacrament of Confirmation.

In the event that you need to request a new original Baptismal Certificate from your student’s parish of Baptism, please include notations (First Communion / Reconciliation dates). The new certificate can be mailed to Holy Family Catholic Church (Attn: Michelle Romero) at 6150 Pershing Ave., Fort Worth, Texas 76107 OR the parish of Baptism can issue the certificate to the parent for submission to HFCC. Please note that Holy Family must keep this certificate on file for our parish records and it will not be returned to you.
