This form was written for the 2021 photo contest and is being edited to include any form of art.

Please submit any art here you wish.

This form is for the photographer who took the pictures submitted for the contest, to consent to their photo being used in the photo submitted. If multiple photos are submitted, you only need to sign one form per contest.

- Submissions without model releases will be disqualified
