Volunteering Acknowledgment

By signing this form, I agree that I am participating in a DICK’S Sporting Goods volunteer event and understand that I will not be paid for my time volunteering.

Also, I understand that my decision regarding whether to volunteer will not impact my employment with DICK’S Sporting Goods in any way and that I am not required to volunteer as part of my employment with DICK’S Sporting Goods.

I hereby grant to DICK’S Sporting Goods, Inc. and its affiliates, designees, licensees, and/or assigns the irrevocable, perpetual, worldwide, sublicensable, and transferable right and license to use and publicize my name, initials, signature, image, likeness, voice, performance, biographical information, and statements (“Identity”) as captured in connection with the volunteer event for any purpose and in any media, including, without limitation, on social media, (collectively, “Materials”) without notice to me and without further compensation. Further, I acknowledge and agree that DICK’S will not be responsible for any unauthorized third-party reproduction or use of or my Identity. 

I, for myself and on behalf of my heirs, spouse, assigns, personal representatives and next of kin (“RELEASORS”), HEREBY RELEASE, WAIVE, DISCHARGE, AND AGREE NOT TO SUE Dick’s, their officers, employees, agents, directors, shareholders, sponsors, insurers, owners of the premises, and/or others associated in any way with my participation in the activity (“RELEASEES”) FOR ANY AND ALL CLAIMS, CAUSES OF ACTION, DAMAGES, LOSSES, PRODUCT CLAIMS AND ANY OTHER ALLEGED LIABILITY that the Releasors may have WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, OR LOSS OR DAMAGE TO PERSON OR PROPERTY AS A RESULT OF MY INVOLVEMENT OR PARTICIPATION IN THE VOLUNTEER EVENT OR THE USE OF ANY EQUIPMENT OR FACILITIES DURING MY PARTICIPATION, WHETHER ARISING FROM THE NEGLIGENCE, GROSS NEGLIGENCE, or RECKLESSNESS OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law. I UNDERSTAND AND AGREE THAT THIS IS A COMPLETE RELEASE AND DISCHARGE OF ALL CLAIMS AND RIGHTS I, ON BEHALF OF MYSELF AND THE RELEASORS MAY HAVE AGAINST RELEASEES AND THAT NO ACTION WILL BE TAKEN BY OR ON BEHALF OF RELEASORS WITH RESPECT TO ANY SUCH RIGHTS.   I, on behalf of the Releasors, hereby RELEASE, DISCHARGE, AND AGREE NOT TO SUE Releasees from any and all claims and demands arising out of or in connection with the use, alteration, distortion or illusionary effect or use in any composite form of my Identity, including without limitation any and all claims for libel or invasion of privacy.

I further agree to INDEMNIFY, HOLD HARMLESS, AND DEFEND in any action or proceeding, Releasees from and against all Claims for or relating to my participation in the activity, my use of any equipment or facilities during the activity, or for my failure to comply with the terms of this Agreement, regardless of whether the act or omission complained was caused in whole or in part by the negligence in any form of Dick’s or Releasees.

Student-Athletes: If I am a student-athlete at the high-school or collegiate level, I acknowledge and agree that, (i) I am responsible for my own high-school eligibility, collegiate eligibility and/or amateur standing,  (ii) I am aware of, and agree to comply with, all rules, regulations, and bylaws of my state association and of any national federation responsible for administering or regulating interscholastic athletics, and of my educational institution, conference, the NCAA or other regulatory authority, that may be applicable to my participation in the event (collectively, “NIL Rules”), (iii) I understand the consequences of any failure to comply with the NIL Rules, including but not limited to, loss of my eligibility to participate in future athletic contests in any sport at the high school, collegiate or other level, and (iv) I waive any right to inspect and/or approve the Materials that may be used in connection with my Identity or the use to which said Materials may be applied.

Pittsburgh Cares Terms & Conditions: I wish to volunteer my services to various community service organizations through Pittsburgh Cares, a Pennsylvania non-profit, charitable corporation.  

In consideration of Pittsburgh Cares providing me with the opportunity to perform some volunteer activities and intending to be legally bound hereby, I waive, release, and forever discharge Pittsburgh Cares and its directors, officers, members, employees, and agents from any and all liability, causes of action, suits, proceedings, damages, judgments, claims and demands whatsoever arising out of my participation as a Pittsburgh Cares volunteer or in any Pittsburgh Cares related activity.

Moreover, I am aware that there are inherent risks of injury to myself, my property and third parties arising from such volunteer activities and I voluntarily assume all such risk and damage arising therefrom, and intending to be legally bound hereby, will hold Pittsburgh Cares and its agents, servants, and employees harmless from any and all liability, actions, causes of action, claims and demands of every kind and nature whatsoever which I now have or which may arise out of or in connection with my volunteer services.

I understand that representatives of Pittsburgh Cares and/or the news media may occasionally take photographs or shoot film footage of volunteers at project sites and I give my permission for my image to be used by Pittsburgh Cares in occasional publicity materials.
