Enacted Federal legislation designed to protect personal privacy, precludes the disclosure of confidential information without the written consent of an individual. The report which you and your counselor at the career development center prepared and which you release to us remains a confidential document, not only by Federal law but also by the regulations of the various centers.
Experience has indicated that the career development center report has its greatest usefulness when it is in the hands of those members of the Commission on Preparation for Ministry who are assigned as your Care Team. After you have become an Inquirer of Pittsburgh Presbytery, the Care Team and the Commission need the reported information so they can help you make greater use of your strengths and help you overcome or compensate for weaknesses. That I may be authorized to release the report to these persons, will you please sign the release below. Without such authorization by you, the Commission on Preparation for Ministry will not be able to help you.
