Let's-Get-PhysiCADs 2026: Road Trip! Registration Form

Hey there! Please fill up this registration form to purchase your slot for our Let's Get PhysiCADs classes from March 4, 2026 to March 27, 2026 in Ateneo de Manila University. The classes will be held at the following venues:

You will be receiving a confirmation email for your registration. Note that you will have to PRESENT this email and your ID at the registration table before the dance classes begin. 

NO CONFIRMATION EMAIL AND ID, NO ENTRY.

We will be accepting walk-ins for Ateneans (For outsiders, please register at least 24 hours before the day of the class/es you wish to attend and if you are to attend MONDAY classes, submit by Friday 12pm of the previous week).

House Rules & Reminders
1. Tickets are non-transferable and non-refundable.
2. Stacking of Promos/Discounts is strictly prohibited.
    Ex. Availing of ANY Promo only applies to Solo Class purchases
    Ex. Using a 3-Class Bundle in which one class makes use of a 5-Buddy Bundle is not allowed
3. The use of more than one (1) Promo/Discount per Form Response is strictly prohibited. Discounts for partner organizations only apply if you select ONE class.
4. For participants under Partner Organizations, you may NOT avail of a Bundle and/or Buddy Promos.
5. For non-Ateneans, please prepare a valid ID to be used upon entry to the campus & class venue.
6. If you are feeling any COVID-19 or flu-like symptoms, please do NOT attend the class.
7. Forms of smoking and kinds of alcoholic beverages are strictly prohibited.
8. Front-of-House will be tracking participants’ attendance, please make sure to pass through the Registration Booth prior to entering the venue.
9. In the case that participants cause commotion and/or violate the basic house rules of the event, organizers have the right to escort them outside the event.
10. Only the class attendees, PhysiCADs Core team and specified personel are allowed inside the venue.
 

Additional Guidelines
1. Should you like to avail different solo classes or packages more than once, you may simply fill up the form again.
2. To ensure that attendees have a quality class experience, we are limiting the Jotform registrants per class.
3. We highly encourage walk-ins! However, the availability for walk-in slots is limited based on the space availability of each class.
