Items you will need to upload to complete this form:

The name and number of your veterinarian
An uploadable PDF of each pet's rabies vaccination.
Dogs only: current license number and expiration date.
If you're renting, please obtain a letter from the owner stating permission to have a pet in the unit.
$25 per pet registration fee, check or money order made payable to PLCA.

All pets must have current vaccinations to be allowed on
Paradise Lakes Condo Association property.
