I authorize Prismatique Maven Trained Professional to perform Eyebrows & Lashes, Facials, or Nail Treatments ( Nail Care & Enhancements) procedures, in a private Mobile/ Home Visit requested location.

I realise that I will be required to pay a deposit, sign form(s) and/or waiver(s) to the necessary service(s), and respond to questions related to the service(s), I have requested.

 This agreement (the "Agreement") is established an entire understanding between the Client and Prismatique Maven on the date of signature.

It supersedes all prior and simultaneous agreements between the parties. The only way to add or change this agreement is to do so in writing, please put a check mark by each term

Both sides agree to the following in exchange for this mutual agreement:

All remaining balance payments are due upon arrival of service for Mobile / Home Visit service, before the performance of services.

Prismatique Maven is responsible for the payment of all materials used in the performance of services to the customer.

You are welcome to bring your own nail art, but you will be charged for the application and time spent applying it.

 

All prices reasonably price with the breakdown service price:

The cost of nail treatment services ( nail care or nail enhancements) is $60. :-

[$15 for set-up, $15 for clean-up, and $20 for mileage, & $10 movement of equipment ,furniture and lighting to transport].

OR The cost of eyebrows, lashes, and facials services is $70. :-

[$15 for set-up, $15 for clean-up, and $20 for transportation and mileage, with an additional $20 for movement of heavier equipment, furniture, massage table, cushions, and lighting to transport].

An itemized estimate via your customer profile email shortly after booking of appointments.

An itemized invoice will be sent via your customer profile email after each service.

All services require a deposit via Bank Transfer, Cash OR Pay Pal.

What's apps for Bank Transfer details must be paid 3-5 days before the appointment.

 The Prismatique Maven professional will be working from a checklist and need to position items in a methodical manner for your service, the Prismatique Maven professional will set up all equipment, furnishings, products, and materials themselves.

To prepare for service, you will be requested to wash your face, hands, and feet with an antibacterial soap/cleanser. Before the service, a set of photos of your face, hands, or feet will be taken.

Along with your Prismatique Maven professional, you will be given a copy of the inspection section of the checklist to sign. If you require a copy of any photographs or the scan/ pdf document checklist, you will receive them via email.

All images taken are for the records and protection of Prismatique Maven, as well as to leave your private residence in the same condition as before the services.

so Prismatique Maven can retain yours & their property in the same condition it was recieved and delivered in.

•  Must have access to water.
• Must have access to electricity for Services, in case power supplier goes out before end of service .
• Before and throughout the appointment, any youngsters [Children]and extra adults who could be a distraction must be kept out of the restricted area during your appointment.
• You, your youngsters [Children], and extra people in the home/mobile site should NOT handle any of Prismatique Maven products or equipment, unless told to so. You will be charged for anything that is broken or goes missing.
• All Pets Dog/ Cats or any pets that can be a distraction MUST be PUT AWAY before and during the appointment.
• Application of Gel Polish/Hard Gel should be done away from direct sunlight. Examples "Gallery/ Patio" during service, reason application brushes cure or gum up.
• Acrylics must be done in a ventilated area.
• Will not tolerate verbally or physically abusive behavior.

Confidentiality

Prismatique Maven acknowledges that in order to operate your service securely, they may obtain or have access to information relating to the Your (Customer's) medical information, past or current. 

Prismatique Maven commits to keeping the Client's privileged data and all forms of it confidential.

 

Acceptance of Terms:

I have read, understand and completed this Terms, Conditions & Guarantee  thoroughly and agree to all Compliance .

I have considered legally bonded to these terms waiver(s) in order to accompany services .

The services I receive here are voluntary, and I release this business " Prismatique Maven"  from liability and assume full responsibility thereof.
