    I, the undersigned hereby agrees to employ MJPS, LLC to conduct my skip trace investigation which is confidential, and I agree to hold the same confidence except for legally permissible reasons in compliance with state, local and federal laws and regulations. The information provided will not be used to cause emotional or physical harm to the subject I agree not to use the information for any criminal act, harassing or stalking. As the client I assume any and all personal responsibility for any violation of the provisions or any damage arising from my actions. The goal of this skip trace investigation is to ascertain the best known location for a person. I agree MJPS, LLC and owner will not assume any responsibility for any facts discovered during the investigation, now or in the future. I understand skip trace & investigative work is not magic, it is understood that the investigator shall use his best efforts to investigate this matter set forth. I understand the results cannot be guaranteed as defined in the terms and conditions. MJPS, LLC retain the right to provide or not to provide you with personal information obtained during the course of the skip trace investigation & will be handled in a one on one basis using our discretion. I agree to indemnify & hold harmless MJPS, LLC and or owner for errors on databases or documents, in addition, we agree not to indemnify, hold harmless it’s agents /employees for any and all damages, actions, causes of actions, demands, acts of terrorism either domestic or international, clients misconduct, misinformation or miscalculations, acts of God, subjects elements, acts or factors which are not in control of MJPS, LLC for now and will not be held liable in the future. Any information we provide you will not be used for any illegal purposes and is punishable by local, state and federal laws.

    *      Authorization of Credit Terms

    I have read and agree to these terms and conditions described here in the Credit Card Authorization. MJPS, LLC agrees to provide the very best service possible. In return, we appreciate and expect your willingness to trust us with your credit card information. Once you click submit, your request is sent to our office and the credit card details are hidden from view. The card information is stored in our secure payment portal (Stripe) for your convenience and peace of mind. By submitting this request, you are confirming that you understand that the charges made are for services rendered and are non-refundable and non-contestable. This authorization agreement shall act as a guarantee of payment for any and all charges incurred for services rendered. We reserve the right to charge your card for any past due balance not paid within our credit terms. Due to the fact that these transactions are transmitted electronically, it is not required that we keep a physical credit card on file for a charge on the card to be valid. We send out statements on the first of each month for unpaid balances. If payment is not received by the 15th of that same month, we will charge the card on file for the total and past due balance. If you are sending a check and its close to the due date, Simply give us a call and we can discuss alternative payment options.

    *      Acknowledgment of Service Agreement

    I have read and agree to these terms and conditions described here in the Service Agreement: We receive this request for service with the true understanding that it is for legal and lawful purposes and will not be held responsible for any and all damages, claims and/or legal actions that may result in carrying out this request. Furthermore, we do not guarantee service will be successful. Once a request has been made, service is deemed in effect and therefore billable. We only provide services once payment arrangements have been made, we accept all forms of payment, Credit Card, Check, Venmo, CashApp, PayPal, and Cash. Payment is due upon receipt of our Invoice. 30 days from the date of invoice, there is a finance charge assessed at 18% APR on the unpaid balance. There are NO REFUNDS and payments made by credit card are uncontestable, and we reserve the right to refuse service to anyone for any lawful reason. By signing this online request for service, you acknowledge that you understand this service agreement, agree to it's terms, and are confirming that you have read our privacy policy that is linked below.
