  • Cancellation of Student’s Pass

  • This form is to be completed by students requesting to cancel their Student’s Pass. For online cancellation, the pass must have minimum 5-days validity. All information given in this form must be accurate and duly completed by student.
  • Important:

    1. Social Visit Pass (white card) will be emailed to student once ICA approves the cancellation.
    2. Do note that the Student’s Pass (STP) will not be valid after the cancellation is approved.

    • I understand that ICA will take approximately 4 working days to approve the cancellation and process the Social Visit Pass (if applicable).
