I, the legal guardian of the above-named camper, authorize the Raise a Child of the Carolinas Summer Camp staff to seek medical treatment for the camper as they see necessary at a nearby approved medical facility. I consent to any x-ray, anesthetic, medical or surgical diagnosis or treatment and hospital care subsequently deemed necessary by a licensed health care provider during the camper’s session. I understand that this authorization is given in advance of any specific diagnosis, treatment or hospital care, and that it is given to provide the camp staff authority to seek medical treatment, and to provide a licensed health care provider the authority to administer this treatment as s/he judges necessary to the above-named child. I accept responsibility for payment of all services rendered; I authorize any medical facility which renders services to release medical information necessary for the processing of insurance claims; and I authorize the payment of insurance claims directly to the medical facility. I understand that whenever possible, the Camp staff will make a good faith effort to contact me or the above-named person(s) before seeking treatment. If this is not possible, I understand that the Camp staff will notify me or my designee as soon a possible of any and all diagnoses and treatments.
The purpose of the above listed information is to ensure that medical personnel have details of any medical problem which may interfere with or alter treatment. I understand that I will be notified in the case of a medical emergency involving my child. In the event that I cannot be reached, I authorize the calling of a doctor and the providing of necessary medical services in the event my child is injured or becomes ill.
I understand that Raise a Child of the Carolinas or Raise a Child of the Carolinas Affiliates will not be responsible for the medical expenses incurred, but that such expenses will be my responsibility as parent/guardian.

If a refund is due, the refund will first be applied to any past due balances (tuition, late fee, trip fees, etc. The RACC registration fees and/or deposits are nonrefundable. All refunds are issued to the original form of payment. Refunds are issued within two weeks of the cancelation date.
*Not attending the program does not entitle you to a refund. When you enroll your child, you are reserving space, time, and staffing whether or not your child attends the program. Please make sure you are only registering for the weeks you will be attending.
If the number of service days used exceeds the amount paid, you will be charged the difference. If you have overpaid, a refund will be issued.
Camp ends at 6pm Anytime after 6pm, i.e. 6:01pm, late fee is an additional $1 per minute
Full weekly tuition payment must be received by Friday of week before the upcoming week of service.
There may be additional cost associated with field trips and activities. Parents will be notified in advance.

I hereby give permission for the transportation of my child for official RACC activities by modes of transportation agreed to by the camp organizers.
In recognizing that me or my child’s attendance and participation in this program may expose me or him/her to risk of injury or harm, I theParent(s)/Guardian(s) hereby waive and release and agree to hold harmless Raise a Child of the Carolinas and its Employees, Sponsors, Instructors, and any other persons associated with the program from actions, claim, or damages that my child may suffer as a result of his/her participation inaforementioned program classes/activities. I further certify that my child is physically fit and capable to participate in the program for whichhe/she is registered. I also give permission for the free and unrestricted use of my child’s name and picture for purposes of publicizing theRaise a Child of the Carolinas' program in any broadcast, telecast or written account of the program.
The RACC and its co-organizers are not responsible for lost or damaged personal property. All scheduled events are subject to change. I understand that no fees will be refunded or transferred unless a child is unable to participate due to an accident or illness per physician orders. Children's photos and quotes may be used for publicity purposes. In case of an emergency, and if a family physician cannot be reached, I hereby authorize my child to be treated by Certified Emergency Personnel (i.e. EMT, First Responder, and/or Physician).
Raise a Child of the Carolinas is committed to offering a fun and safe learning environment. We will not tolerate disruptive behavior, bullying, or violence of any kind. We will maintain an Organization Safety Plan addressing procedures to provide a supportive, safe, healthy, orderly, and equitable learning environment for both students and staff. This plan will also address any issues identified by our stakeholders and issues required by state law. A copy of our current Safety Plan will be provided upon registration.
As Parent/Guardian, I understand and accept the above program expectations and I give my permission for my child to participate in The RACC program.
