Raise Leaders Martial Arts – Day Camp Liability Waiver & Participation Consent

I give permission for my child to participate in the Raise Leaders Martial Arts Day Camp on the date(s) selected on this registration form. I authorize RLMA, LLC (Raise Leaders Martial Arts) staff to provide basic first aid and CPR if necessary and, in the event I cannot be reached, I authorize RLMA, LLC to seek emergency medical care for my child, including transportation by ambulance if needed, and I consent to any medical treatment deemed necessary by licensed medical professionals to safeguard my child’s health. I understand that participation in martial arts training, physical activities, games, obstacle courses, and related camp activities involves inherent risks, including but not limited to slips, falls, collisions, and other accidental injuries, and I voluntarily assume all risks associated with my child’s participation in Day Camp. I hereby release, waive, and discharge RLMA, LLC (Raise Leaders Martial Arts), its owners, officers, employees, volunteers, independent contractors, and agents from any and all claims, liabilities, damages, or causes of action arising out of or related to my child’s participation in Day Camp, including claims resulting from ordinary negligence. I understand that RLMA, LLC is not responsible for lost, stolen, or damaged personal property brought to Day Camp, and I acknowledge that it is my responsibility to maintain appropriate health and accident insurance coverage for my child, as RLMA, LLC does not provide medical insurance coverage for participants. I grant permission to RLMA, LLC to use photographs or video of my child taken during Day Camp for promotional, marketing, and social media purposes.
