Raise Leaders Martial Arts – Spring Break Camp Liability Waiver & Participation Consent (March 16–23, 2026)

Participation Consent
I give permission for my child to participate in Raise Leaders Martial Arts Spring Break Camp taking place March 16–23, 2026, on the date(s) selected on this registration form.

Medical Authorization
I authorize RLMA, LLC (Raise Leaders Martial Arts) staff to provide basic first aid and CPR if necessary. In the event I cannot be reached, I authorize RLMA, LLC to seek emergency medical care for my child, including transportation by ambulance if needed, and I consent to any medical treatment deemed necessary by licensed medical professionals to safeguard my child’s health.

Assumption of Risk
I understand that participation in martial arts training, board breaking, weapons training, parkour and acrobatics, sparring (boxing and wrestling), games, obstacle courses, field trips, and related camp activities involves inherent risks, including but not limited to slips, falls, collisions, physical contact, and other accidental injuries. I voluntarily assume all risks associated with my child’s participation in Spring Break Camp.

Field Trip & Transportation Consent
I grant permission for my child to participate in off-site field trips and activities associated with Spring Break Camp. I understand that transportation may be provided by RLMA, LLC staff, rented vehicles, chartered transportation, or by walking to nearby locations when applicable. I acknowledge that off-site participation carries inherent risks, and I voluntarily assume those risks.

Behavior & Dismissal Policy
I understand that Raise Leaders Martial Arts maintains a structured, respectful, and safe training environment. Campers are expected to follow all instructor directions, demonstrate respectful behavior toward staff and fellow campers, and adhere to safety guidelines at all times. If a camper engages in unsafe, disruptive, or inappropriate behavior, I understand that I may be contacted for immediate pick-up. In cases of repeated or serious misconduct, RLMA, LLC reserves the right to dismiss a camper from Spring Break Camp without refund to ensure the safety and positive experience of all participants.

Release of Liability
I hereby release, waive, and discharge RLMA, LLC (Raise Leaders Martial Arts), its owners, officers, employees, volunteers, independent contractors, and agents from any and all claims, liabilities, damages, or causes of action arising out of or related to my child’s participation in Spring Break Camp, including claims resulting from ordinary negligence.

Insurance & Personal Property
I understand that RLMA, LLC is not responsible for lost, stolen, or damaged personal property brought to camp. I acknowledge that it is my responsibility to maintain appropriate health and accident insurance coverage for my child, as RLMA, LLC does not provide medical insurance coverage for participants.

Media Release
I grant permission to RLMA, LLC to use photographs or video of my child taken during Spring Break Camp for promotional, marketing, and social media purposes.
