MEDIA CONSENT AND RELEASE

As parent/guardian, I understand that promotional pictures and videos (individual and group) may be taken during tournaments, practices, workshops, life skills classes, and other Reach One Youth Organization, Inc./Reach One Sports/Reach One Elite and Affiliates events. I give permission for my or my son’s/daughter’s pictures and/or videos to be used in promotional materials (newsletter, web page, calendars, PowerPoint, video, audio, etc.) highlighting Reach One Youth Organization, Inc./Reach One Sports/Reach One Elite and Affiliates events. Furthermore, I understand and give permission to Reach One Youth Organization, Inc./Reach One Sports/Reach One Elite and Affiliates to use photographs, videos, audio and other media exposure/release for sports-related recruiting on behalf of my child. I release the staff, volunteers, etc. of the Reach One Youth Organization, Inc./Reach One Sports/Reach One Elite and Affiliates from any liability connected with the use of my or my child’s pictures, videos or voice recording as part of any of the above or similar activities. *

REACH ONE SPORTS LEAGUE REFUND POLICY 

I, THE UNDERSIGNED PARTICIPANT, AFFIRM THAT I AM OF THE AGE OF 18 YEARS OR OLDER, AND THAT I AM FREELY SIGNING THIS AGREEMENT. I CERTIFY THAT I HAVE READ AND FULLY UNDERSTAND THE ABOVE WAIVER AND RELEASE OF LIABILITY, TERMS AND CONDITIONS. I FULLY UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS, INCLUDING THE RIGHT TO SUE, BY SIGNING THIS WAIVER VOLUNTARILY.
