I understand that this procedure is a permanent change to my skin and body.
I understand that there is a possible risk of an allergic reaction to the tattoo ink.
I understand that I need to take care of the tattoo by following the instructions and using the aftercare kit provided to me by Redeemed Ink & Co. Improper care could lead to unwanted results or risk of infection. Aftercare is also provided on artists website (www.redeemedinkandco.com)
I understand results vary from person to person.
I understand one or multiple touch ups may be needed after healing. First touch up is complimentary and any additional touch ups needed are at an additional cost.
I am NOT pregnant or breastfeeding at time of scheduled appointment.
I certify that I am 18 years of age or older.
I agree that the design is drawn to my satisfaction and if uncomfortable with design, artist or establishment that I'm encouraged to have it redesigned or decline tattoo.
I confirm that the information I provided in this document is accurate and true.
I hereby release the artist at Lisa Marie Oliver LLC, the establishment DBA Redeemed Ink & Co and the owner of all legal responsibility of the tattoo and health of the client. I understand the establishment is licensed in the State of Florida to legally tattoo.
I certify that I have received an estimate for the service at Redeemed Ink & Co and that all payments are final and due in full at the end of every tattoo session.
I certify that I know all needles used for my tattoo are single use, pre-packaged and pre-sterilized. I understand I am able to check all materials used for the tattoo before the session has started and am able to have information about the tattoo materials explained to me.
I understand that before my tattoo has started that all my questions have been answered and I am comfortable with the tattoo design, artist and establishment.
