"We've Got Spirit" VBS Registration Form

DEADLINE: The deadline for registration to guarantee a T-shirt is May 18th, 2025. After that, online registration will close - you will still be able to register on-site, but you will not be guaranteed a T-shirt.

PLEASE NOTE: If you are signed up for our Summer Camp program via our Preschool, VBS is included with that registration. You do not need to fill out this form if you are already signed up with them. More information about our Summer Camp can be found here.

VBS Registration FeeThe online registration fee includes a T-shirt as well as helps us cover supplies and food for the entire week. The fee is $25 per student. Special discounts apply if your family has 4 or more students (fee is 50% off beginning with the 4th student in the same family. Cannot combine registrations with other families.) Online Registration CLOSES on May 18th, 2025. After that date, only on-site registration is available - on-site registration price is $25 and does not guarantee a T-shirt. Payment can be made online. 1. I am authorizing my child(ren) to participate in Lamb of God Lutheran Church's 2025 Vacation Bible School.
2. All fees are non-refundable.
3. I authorize the church staff, bible study conductor, and/or volunteer personnel to conduct first aid and medical care in the event of an emergency situation. I understand that the staff will contact the person listed under "Emergency Contact" for my child(ren) should the need arise - and will call 911 if there is a true emergency situation. I agreed to pay for all the medical care expenses and costs in a given situation that medical care is needed.
4. I release the church from any liabilities that might happen during the activity and hold them harmless in the event of damages, injuries, or accidents.
