    Tattoo Enquiry Form

  • Thanks for your interest in getting tattooed by me.

Please fill out this form clearly and completely, including all requested info and photos. Incomplete or vague submissions slow down the booking process and may mean you miss out on your preferred date(s).

Do not submit multiple forms - it can cause delays and miscommunication.

I respond to all enquiry forms within 3-5 business days. If it's been longer, check your junk/spam folder first, then send a follow up.

For general questions, use the box at the end of the form.

Thanks for your time and cooperation,
    Rochelle.

  • Client Information

  •  . .
  • Design Information

  • Be clear and specific about what you want included (or excluded) in your design. Include details like:

    • Specific text (names, dates, words, quotes).
    • Theme or subject matter(s) (e.g. flower or animal types, Greek gods/goddesses)

    Examples of clear descriptions:

    • "Bird and flowers - no preference of types used"
    • "A common sparrow with daisies and poppies"
    • "Greek mythology - artist's choice"
    • "Ocean theme - no whales"
    • "Name 'Omega' and date '2020'"

Do not submit vague requests like "flowers", "sleeve", "name and date".

  • Reference Photos (optional)

Upload any reference photos that help communicate your preferred style, subject matter, or colour preferences.

These will be used as inspiration only, not copied - unless it's a portrait of a specific person or pet, print (hand/foot/paw), or handwritten text.

For portraits, make sure the images are clear, high-quality, and match the angle you want (e.g. don't send a side profile if you want a front-on piece).

If you're after a direct copy of someone else's tattoo or artwork, that's your choice - but I'm not the artist for you.

  • Location Photos

Upload clear photos of the area(s) you want tattooed. These help me design specifically for your shape and size, reducing the need for last-minute design revisions on the day.

    Requirements:

    • Best taken by someone else.
    • In natural, indirect sunlight or under clear lighting.
    • You must be standing relaxed and naturally.
    • Photos must be taken flat/parallel to the area - not angled.

If you don't want the full area tattooed, please upload a duplicate copy marked with your preferred boundaries (top/bottom/widest points).

    Can't Take Good Photos?
Let me know in the 'Other Questions' box at the end of this form, in case we need to arrange a time for me to take them in person at the shop.
