2024-25 D6710 Expense Reimbursement Request

This form is to be used for REIMBURSEMENT of expenses incurred on behalf of Rotary District 6710 during the 2024-25 Rotary Year (between 7/1/2024-6/30/2025).

• NEW THIS YEAR: Reimbursement should be requested within 14 days of expense. Exceptions will need prior approval from DG Dale.
• Receipts must be attached. 
• DG Dale Leatherman has final approval of all expenses.
• Special situations or large expenditures must be discussed with and approved by DG Dale PRIOR to the expense and the submission of this form.
• Please remember that we are all volunteers and allow ample time for processing. 
• Please note: Incomplete forms or lack of receipts may delay processing. 

DIRECTIONS FOR COMPLETING THIS FORM

There are 4 sections for you to list your expenses:

• Expenses Incurred for Your Office or Committee: for the operation of your position such as supplies, printing, postage, etc.
• Expenses Incurred for an Event that you are attending or coordinating such as Lodging, Meals (if not included in Registration), Registration, Room Rental, etc. 
• Mileage Reimbursement for Personal Vehicle Usage: for pre-approved travel. IRS reimbursement for volunteers is .14 per mile. 
• Transportation Expenses: such as Airfare, Cab/RideShare, Car Rental, Parking, Tolls, Train

DIRECTIONS

• Enter the date (mm/dd/yy), amount ($xx.xx) and other  requested information. The totals will be calculated and entered automatically.
• Be sure and attach receipts for all expenses.
• There is a NOTES section at the end of the form you can use to relay any information about your expenses.
• If you save the form to complete it later, an email will be sent to you with the link to your uncompleted form.
• When you click the submit button, you will receive a copy of your submission and it will also be forwarded to DG Dale for approval. Upon approval, you and District Treasurer Kenneth Hatzakorzian will be notified, and Ken will forward a check to you at the address listed below.
