Acknowledgment and Assumption of Risk: I hereby acknowledge that participation in training activities, including but not limited to sprinting, weightlifting, and related physical exercises (hereinafter referred to as “activities”), involves inherent risks of physical injury. These risks include, but are not limited to, muscle strains, sprains, fractures, or other physical harm. I fully understand and voluntarily assume all risks associated with participation in these activities.

Release of Liability: In consideration of my participation and/or allowing my child to participate in these activities, I, on behalf of myself and/or my child, hereby release, discharge, and agree to hold harmless RubrixQube Sports Performance LLC and its employees, agents, and representatives from any and all claims, liabilities, or causes of action arising out of or related to any injury, illness, or property damage that may occur as a result of participation in the activities.

Payment and Cancellation Policy: I understand and agree to the following payment and cancellation policy: Payment for training sessions must be made in advance (prior to the start of the training session). If a session is canceled without at least 24 hours of notice, payment for that session is non-refundable and cannot be credited toward future sessions.

Consent for Emergency Medical Treatment: In the event of a medical emergency, I authorize RubrixQube Sports Performance LLC to seek necessary medical treatment for myself/my child. I agree to be responsible for any medical expenses incurred as a result of such treatment.

Rules and Expectations: I understand that I/my child is expected to adhere to all safety guidelines and instructions provided by RubrixQube Sports Performance LLC and its trainers. Failure to do so may result in suspension or termination of participation without refund.

Photo/Video Release: I grant permission for RubrixQube Sports Performance LLC and its trainers to use photographs or videos of myself/my child taken during training sessions for social media and promotional purposes. 

Governing Law: This waiver and release shall be governed by the laws of the Commonwealth of Virginia.

Acknowledgment of Understanding: I have read this waiver and release of liability agreement and fully understand its terms. I understand that I am waiving certain legal rights by signing it, including the right to sue.
