  • Agreement: The person(s) whose signature(s) appear on this contract, known as "Client", agree that (EVELYN MARIE BEAUTY) Name known as "EVELYN RUSSO", shall provide services to apply the best of her abilities, in the manner described in this document. This is a binding contract which incorporates the entire understanding of the parties, and any modifications must be in writing, signed by both parties, and physically attached to the original agreement. Client assumes responsibility for all collection costs and legal fees incurred should enforcement of this contract be broken.

    Liability: Any allergies should be reported prior to application. The Client agrees to release the Artist from liability for any complications due to allergic reactions. Method of Cancellation/Rescheduling: In the event of a cancelled or rescheduled event, (EVELYN MARIE BEAUTY) requires a notification inwriting 30 days prior to the original event date. If cancellation happens after the 30 day deadline the Client is responsible to pay the full amount.

    Client Responsibilities: Itis the Clients responsibility to provide a safe and adequate working environment for the Artist. The location must include: Plenty of lighting (ring light provided by artist) Enough working space (professional chair provided by artist) Outlets

    Table for product and tools Coordinate with venue to ensure the above is ready Everyone receiving services must have clean fully dry hair or a blowdry fee of $75.00 will be added to their balance due. Charged via cash, Venmo, Zelle, Cashapp or Paypal. Party Requirements: Minimum of 6 people and a maximum of 8 people. Evelyn Marie Beauty is an individual traveling stylist. To ensure services are done on time, appropriate start time will be chosen by the artist and a headcount exceeding 8 will require a second artist. Everyone is expected to be present at the booked time, and any individual not on site by the end of services is a missed appointment and their full balance is due. Parties of 5 or less will be required to rent the artists bridal space at Heyday Collective in Lakewood. Payments: A retainer fee of $100.00 is due along with your contract at the time of booking; this payment is non- refundable and must be paid in order to secure your event day. The remainder of your balance is due in CASH ONLY. Payment will be collected before services begin on the event day. This is to ensure your artist is paid before paying parties are busy with the on-site photographer or preparing to leave.

    Travel: A mileage fee will be charged as follows: $1.00 per mile exceeding 30mi from Grafton Ohio. Any parking or valet fees will be added to travel fee.

    Photography: Photos or videos may be taken for promotional use and advertising purposes online and printed material. Full credit will be given to any professional photos and videos taken by photographers and videograhers

  • I have read, understand and agree to the terms and conditions of this Agreement. Each person signing as Client below shall be fully responsible for ensuring that full payment is made pursuant to the terms of this Agreement.
