Activities Waiver

Fill out one waiver per household by a legal parent/guardian/adult. This waiver is valid for the entire calendar year.
Covers ALL Sandy Cove Activities Including, but not limited to:
- Big Swing (ages 12+)
- Zip Line (ages 12+)
- Escape Room (ages 12+)
- Rock Wall (ages 8+)
- Cargo Net (ages 8+)
- Team Building (ages 8+)
- Archery/Slingshot (ages 8+)
- Archery Tag (ages 8+)
- Water Sports (ages 6+)
- Pedal Carts (ages 9-)
- Pony Rides (under 100lbs)
- Marina and Pools (all ages)
Please Note: Activities are not always offered – signing a waiver does not guarantee its availability during your stay. Guests may also be refused service at an activity due to factors that affect safety.

Terms and Conditions
• I am aware that participation in the listed activities can be dangerous, and I give permission for the listed participants to voluntarily enter these activities with knowledge of the danger involved and hereby agree to accept any risk, including injury and/or death, or loss.
• I agree that the listed participants will follow any posted or spoken rules at each activity site, including obedience to any age/weight requirements.
• I certify that the listed participants have no medical or physical conditions that could interfere with participation or else assume any risks created or impacted by such conditions. I further certify I have adequate insurance to cover any injury or damage caused or suffered while participating and agree to bear any costs of such injury or damage myself.
• I hereby assume full responsibility for the risk of bodily injury, death, or property damage for listed participants due to the actions of Sandy Cove or otherwise while participating in one or more of these activities. I agree to indemnify, hold harmless, and defend Sandy Cove, and I waive all claims for liability against Sandy Cove and its staff, vendors, and contractors.
• I acknowledge any claim or dispute arising from or related to this indemnity agreement shall be settled by mediation and legally binding arbitration if necessary, in accordance with the Rules of Procedure for Christian Conciliation of the Institute for Christian Conciliation, a division of Peacemaker® Ministries (www.peacemakerministries.org). Judgment upon an arbitration decision may be entered in any court otherwise having jurisdiction. I understand that these methods shall be the sole remedy for any controversy or claim arising out of this agreement and expressly waive my right to file a lawsuit in any civil court against one another for such disputes, except to enforce an arbitration decision.
• I have carefully read this agreement and fully understand its contents. I understand this is a release of liability and a contract between myself and Sandy Cove Ministries and its affiliates. By signing it, I am giving up all claims of any kind for injury or loss as it relates to the listed participants in exchange for permittance to participate in listed activities and do so of my own free will. I am signing this form for myself and/or on behalf of the listed participants, all of whom are members of my immediate family or under my legal care and I have authority to sign this form on their behalf.
