By signing below, I acknowledge and agree to the following:
1. Assumption of Risk
I understand that participating in fitness classes, exercise programs, stretching, strength training, cardiovascular activities, and related activities involves inherent risks, including but not limited to muscle strains, sprains, falls, illness, injury, permanent disability, or death. I voluntarily choose to participate and assume all risks associated with my participation.
2. Medical Responsibility
I certify that I am physically able to participate in these activities. I understand it is my responsibility to consult with my physician before beginning any exercise program if I have any medical concerns. I agree to immediately stop participating and notify the instructor if I experience pain, dizziness, shortness of breath, or any other concerning symptoms.
3. Release of Liability
To the fullest extent permitted by law, I release, waive, and discharge Sara Stevens Fitness LLC, Gathered Moms Co., the class instructor(s), volunteers, assistants, employees, independent contractors, and the host venue/property owner from any and all liability, claims, demands, damages, costs, expenses, or causes of action arising out of or related to my participation in this fitness class, including claims resulting from ordinary negligence.
4. Venue Release
I understand that this class is being held at a venue that is providing space for the event. I release the venue, its owners, employees, and representatives from liability for any injury, loss, damage, or accident that may occur while attending or participating in this event, except where prohibited by law.
5. Child Supervision
I understand that childcare is not provided during this event.
If I bring my child(ren), I acknowledge that I am solely responsible for supervising and caring for them at all times before, during, and after the class. I understand that neither the instructor, volunteers, Gathered Moms Co., Sara Stevens Fitness LLC, nor the venue assumes responsibility for supervising children or ensuring their safety.
I accept full responsibility for my child(ren)’s actions and well-being while attending the event.
6. Personal Property
I understand that I am responsible for my own personal belongings. The instructor, Gathered Moms Co., Sara Stevens Fitness LLC, and the venue are not responsible for any lost, stolen, or damaged personal property.
7. Photography & Media
I grant permission for photographs or videos taken during the event to be used for promotional, educational, or social media purposes by Sara Stevens Fitness LLC and Gathered Moms Co.
8. Acknowledgment
I have carefully read this waiver, fully understand its contents, and voluntarily agree to its terms. I understand that by signing this document I am giving up certain legal rights.
This waiver shall remain in effect for all classes, events, and activities hosted by Sara Stevens Fitness LLC and Gathered Moms Co. for a period of one (1) year from the date signed, unless superseded by a revised waiver.
