By signing I understand that all deposits left with The Fox & Fable are nonrefundable! Deposits are able to be rolled over if you give at least 72 hours notice to give us ample time to fill your spot! We only allow one reschedule on clients end. If more than one reschedule is requested you will need to leave a new deposit. If not I agree to forfeit my deposit amount. By signing I also understand that appointment dates may change on the artist ends at any time due to last-minute doctors appointments/travel dates and that my deposit will roll over onto the reschedule date discussed. By signing I understand that Shenise reschedules frequently due to her disability, IVF appointments and or lack of child care on occasion. Sometimes a reschedule will be a few months out. By signing I understand that the deposit amount is the artists drawing fee and is non refundable. I agree to look over the rules and regulations of the tattoo studio on their website and agree and understand all rules and regulations. I also understand that no changes can be made to the design after submitting this form. By signing I understand that visitors are not allowed into the studio at all and under no circumstances. You will need to come to your appointment alone. If you have diabetes or any skin conditions please be sure to bring in a doctors note stating that it is OK for you to be able to get tattooed with your condition. *
