On submission of this application, I hereby confirm that I am fit to train and have been certified by a medical doctor as not suffering from any medical condition/s which hinders me from taking part in athletics training sessions. I will not hold the club, its committee members or the coaches/leaders/helpers responsible for any damages, loss, injuries or death. In the event of illness or accident, I give permission for medical treatment to be administered where considered necessary by a suitable medical practitioner and/or hospital. Payment of competition fees does not guarantee participation - athletes may be blocked from taking part if they do not abide by the club's and Athletics Malta's rules, such as the obligation to provide assistance during competitions.
I give the club consent to use any photos/videos of me on the club's online platforms and media.
There may be instances when transport by cars or any kind of larger vehicle (minibuses, buses, etc) may be required. In such instances I give consent for the child to use such transport, provided that the driver/s are licensed and insured, and are using a licensed and road worthy vehicle.
