  • Required Documents

    The following documents will be required in order to complete your application though you may complete this form and submit documents later if you do not have them available at this time. 
    1. Age Verification - Birth certificate (preferred), a passport, an I-94, a transcript, OR a report card.

    2. Grade Level Verification - Transcript, promotion certificate, or the student’s most recent report card.

    3. Verification of Residence (2 forms) - A document with the parent’s, guardian’s, or caregiver’s name and address on it. Acceptable options include:
      • A utility bill dates within 45 days: PG&E, EBMUD.
      • In combination only, automobile registration and automobile insurance.
      • Homeowner’s/renter’s insurance policy.
      • Lease agreement with owner’s documentation.
      • Property tax statement.
      • Official letter from a social services/government agency within 45 days.

    Additional documentation may be requested. If you’re struggling with acquiring any of these documents, please contact us for assistance.

    Transitional families who reside at a non-permanent residence, such as a motel/hotel, shelter, car, or are doubled up with relatives or friends due to hardship and who are unable to establish/document permanent residency can contact the school office for residency verification and assistance.

    1. Verification of Custody - Parent’s, guardian’s or caregiver’s passport, driver’s license, or official ID Card in conjunction with documentation in form of Birth Certificate, guardianship papers or court documents.
