Please complete this form to indicate your Commitment to Register your child for the upcoming school year, request enrollment of any siblings, or to register your child, if you are a staff member.

To secure re-enrollment and request enrollment for siblings and staff children, this form must be completed. If we do not have this form completed, you forfeit your child’s seat to those applying for the lottery to be held in May.

Please complete one form for each of your children.
If there are no available spots for new siblings, the registrar will notify families that they will be placed on the preferred waitlist.

By answering yes above and signing this Commitment for Re-Enrollment form, I am acknowledging that my child will be attending SABE for the upcoming school year. Should these circumstances change (i.e. moving from area, moving out of state, homeschooling, change in schools), I understand that it is imperative and my responsibility that I notify the school immediately to release my child’s position for those on the waitlist. Enrollment numbers directly impact the financial position of the school and the courtesy of notifying the school is required if you choose not to enroll your child at SABE. I also understand that if I am contacted via telephone or email, that I must reply/respond within 24 hours or I forfeit my child's enrollment. I also understand that i will need to complete the annual registration packet to update all student demographics information and complete any additional forms requested by SABE.
