Complete one submission for each Hometown Hero. Please enter all information exactly how it should appear on the banner and upload the highest-quality photograph available.
Banner submissions must be submitted by April 1st and by October 1st of each year to have them ready by Memorial Day and Veterans Day.

I understand that the total cost of the banner is $120.00 and payment will be made with submission of the application form. Payment of check or cash, or card must be submitted to the appropriate town clerks office or to Seth Sports Photography, unless prior approved payment has been discussed.
