I also hereby agree and fully understand Shogun Martial Arts LLC has a 30-Day notice required to cancel membership, as per the Membership terms agreed on upon enrollment. If you have a payment scheduled in that alloted timeframe, please be aware that payment must process, and cannot be cancelled.

    If resume date is submitted below, I acknowledge my payment and classes will resume on the date selected. Your tuition rate will not be affected by chosing this option.

    If resume date is not submitted below, you acknowledge cancellation of your program and will be subject to paying the current standing tuition rate upon return (promotions at the time of return are not applicable).
