It is understood that Sketchy Mutts and Underdogs requires a multi-step adoption process prior to fully approving an adoption application for one of our dogs. A Sketchy Mutts facility visit will be scheduled upon approval of this application. After a successful facility visit, a visit to the adopter’s home will be scheduled. If the adopter’s home visit is successful, we will schedule the dog to visit your home for one or more “overnights”. Completion of the trial period will result in either the dog being returned to Sketchy Mutts or a completed adoption agreement. If at any time a representative of Sketchy Mutts feels that this is not the best situation for the dog, the application can and will be voided. Our goal is to place our dogs into forever homes that will best suit their individual needs.
By submitting this Adoption Application, you are attesting to the accuracy and truth of all answers given on this form. If any of the information you have provided proves to be false, this will automatically void this application.
