Volunteers & Donations
The Athletic Association is dedicated to the social, athletic, and spiritual growth of our children, and it operates through the selfless dedication of volunteers. We kindly request that individuals commit to volunteering for at least 5 hours or more throughout the school year (from September to June).

Furthermore, we warmly welcome additional financial contributions! Any donation, regardless of its size, will contribute to the expansion of our association and will be exclusively used to enhance sports programs, acquire better equipment, secure improved facilities, and more. By collaborating together, we can achieve truly rewarding outcomes for our children. Please note that participation is entirely voluntary, and any contribution will be greatly appreciated.
Important Information & Media Consent
Refund Policy: Please note that registration fees, uniform costs, late fees, non-volunteer fees, and donations are non-refundable. Full refunds will only be granted in the event of team cancellation before the commencement of the regular CYO season. No refunds will be provided if volunteer participation, such as the availability of a coach, becomes insufficient after the team entry fee has been paid.

Media Consent: As the parent/guardian of the aforementioned player, I hereby provide consent for participation in interviews, the use of quotes, names, and the capture of photographs, videos, movies, or audio recordings by the SLAA or organizations authorized by the SLAA. I also authorize these same organizations to edit, utilize, and reuse these materials for non-profit purposes. Additionally, I release the St. Luke's Athletic Association, its agents, and employees from any and all claims, demands, and liabilities in connection with this consent statement.

I, as the parent/guardian of the player mentioned above, grant my consent for their participation in all activities related to this sport for the upcoming season. I acknowledge and accept all associated risks and hazards, including transportation to and from these activities. I hereby release the St. Luke's Athletic Association, the Parish of St. Luke's, and any individuals responsible for the player's transportation from any liability, except as covered by accident or liability insurance. To the best of my knowledge, I certify that the player is in good health and does not have any physical conditions that would hinder their participation in this activity.

- I Completed all the necessary fields with accurate information and made a full payment using one of the following methods: check, money order, or online credit card payment to the St. Luke's Athletic Association.
