Describe the situation and the actions of the Apprentice which you feel are noteworthy. Please be as open, honest, and detailed as possible. The JATC will review the report as well as any other applicable information. Should the JATC agree that the actions/behaviors/knowledge of the apprentice exceed the standards and expectations of the program, a certificate of exceptional performance shall be awarded along with an additional copy being placed in his/her apprenticeship file.
