I acknowledge and agree to the terms and conditions outlined for enrollment in the SLOAN STEM+SAT Prep Course. I understand that participation in this program is contingent upon meeting the following requirements: I commit to attending at least six out of the eight scheduled SAT prep classes in the fall. I will submit all supporting documentation (unofficial transcript and prior PSAT/SAT/ACT scores). I understand that regular attendance is crucial for maximizing the benefits of the course and improving my SAT scores. I will inform the course coordinator beforehand if unavoidable circumstances prevent me from attending. I agree to actively engage in class activities, complete assigned studies and/or test questions, and participate in discussions to enhance my learning experience. I acknowledge that I will take the proctored pre-test submission required as it is essential for tracking my progress and providing necessary support. I will adhere to the code of conduct and exhibit respectful behavior towards instructors and fellow participants throughout the course. By signing below, I confirm my commitment to fulfilling these requirements and understand that failure to do so may impact the program's overall effectiveness for myself and other participants. I understand that there are no guarantees of improvement or passing at a certain percentage. I understand that SLOAN STEM maintains the right to refuse participation if this agreement is broken and is not held liable for the physical, emotional, or psychological impact of course participation. I agree not to hold SLOAN STEM+ARTS INC., The Luke Church, or SAT prep course staff representatives liable.

FOR YOUR STUDENT'S ATTENDANCE CONTINGENT UPON ALL DOCUMENTS RECEIVED, PRE-TEST TAKEN BY AUGUST AND PREREQUISITES. IF WE ARE UNABLE TO SECURE A POSITION FOR YOUR STUDENT, WE CAN PLACE YOU ON THE WAITLIST AND OR PROVIDE A PERCENTAGE OF THE PAYMENT TO BE RETURNED.
