By signing below, you acknowledge that the information you have provided is accurate and truthful to the best of your knowledge. You also confirm that you have read and understand the responsibilities and commitments required of a guardian home, including but not limited to:

• Providing daily care, training, exercise, and mental enrichment for the dog.

• Maintaining the dog's health, veterinary care, and insurance as outlined by the breeder.

• Allowing temporary separation for breeding or whelping and ensuring transportation for these purposes.

• Adhering to the breeder's policies until the dog's retirement from the program.

• Understanding that the discounted price of the dog reflects your commitment to fulfilling these responsibilities.

Failure to meet these responsibilities may result in the termination of the guardian home agreement.
