  • Thank you for your interest in an appointment for wellness services with the Spay Neuter Veterinary Clinic of the Sandhills located in Fayetteville, NC!  This request is for vaccinations or other wellness care and NOT for surgery.  For surgery appointment requests please fill out the surgery request form available here.


    Please fill out the information below completely. You can choose the day of the week you prefer for your appointment. 

    SNVC offers wellness appointments as "drop off" appointments which are available multiple days of the week. For drop off appointments made Monday - Thursday, you would drop off between 8:00am-8:30am with pick up later in the afternoon around 3pm. For drop off appointments scheduled on Friday, drop off is between 8:00-9:00 and pick up is at noon. We close at 1pm on Fridays, so all pets would have to be picked up by noon.

    The appointment request form requires you to submit the $25 payment for the appointment booking fee. We are not able to schedule an appointment without the booking fee. If you have money for a booking fee on your account already, please email us at info@spayneutervet.org with the animal information and we will be happy to schedule the appointment for you. If you prefer to pay the deposit in person you can come to the clinic or give us a call, as you will not be able to submit this form. The booking fee goes to cover the price of the physical exam if you show up for your scheduled appointment, but is lost if you do not show up for your appointment or need to change the appointment with less than 10 days notice.  If you need to cancel your appointment with more than 10 days notice and are request a refund for the booking fee, a $5 handling fee will be charged to cover processing fees that we are charged. 

    When we receive the completed appointment request form and confirmation that the payment has processed, we will email you to give you the first available date for your appointment and you will be scheduled for that day. 

    Once you receive the appointment confirmation email, confirm back to us within 48 hours that you would like that date or if you prefer a different date. If you do not let us know within the 48 hours that you would like to change the appointment date, your appointment date will be set and the standard missed or canceled appointment fees apply.

  • In addition to the exam and other fees, intact animals will be charged $25 per wellness visit.  This fee is credited back at the time of surgery if you schedule and pay the deposit for your spay or neuter surgery appointment prior to the end of your wellness appointment.

  • *Please remember to fill out a separate form & submit deposit for each individual pet you have scheduled!*

  • Wellness appointments are all schedule on a drop off basis. For appointments Mon - Thursday, you drop off between 8:30- 9:00am and pick up at 3pm.

    For Wellness appointments on Fridays, you drop your pet off between 8:00-9:00am and pick them up between noon-1:00pm.

  • TO ENSURE SAFE HANDLING FERAL CATS MUST BE BROUGHT IN A TRAP AND MAY REQUIRE SEDATION TO BE SEEN DEPENDING ON SERVICES REQUESTED

  • We offer three types of flea and parasite prevention for your cat:

    • Parasedge Multi - Topical medication that prevents heartworms, and treats fleas for 1 month. Parasedge also treats ear mites and intestinal parasites, such as hookworms and roundworms.
    • Bravecto – Topical medication that prevents fleas and ticks for 3 months with a single dose. Cats should be at least 6 months old to start Bravecto.
    • Nexgard Combo- Topical medications that prevents heartworms, fleas and ticks for 1 month. It also provides deworming against hookworms, roundworms and tapeworms. 
  • With a heartworm test performed by us or proof of negative heartworm test, you can purchase Heartworm prevention for your pet.  We offer a monthly chewable and an injectable heartworm prevention that lasts for 6 months with one shot. You can choose either option or we can help you decide!

  • Available Options for Heartworm Medication:

    • Iverhart Max - Oral monthly chew that prevents heartworms and intestinal parasites, including tapeworms.
    • Parasedge Multi - Topical that prevents heartworms, and treats fleas for 1 month. Parasedge also treats sarcoptic mange and intestinal parasites, such as whipworms, hookworms and roundworms.
    • ProHeart – Injectable heartworm prevention that prevents heartworms and most intestinal parasites for 6 months with just one injection. Dogs must be over 6 months old to receive the ProHeart.     • Nexgard Plus- Oral monthly chew that prevents heartworms, treats roundworms & hookworms , prevents/kills fleas AND ticks. 
  • SNVC Deposit Policy:

    The Spay Neuter Veterinary Clinic of the Sandhills requires a $25 booking fee be paid prior to scheduling any wellness appointment.  The payment is used to cover the cost of the wellness exam when your pet shows up for the scheduled appointment.

    If you need to cancel or reschedule the appointment, please let us know more than 7 days prior to the appointment. If you let us know with less than 7 days’ notice or do not show up for the appointment, you will lose the $25 booking fee paid.  That $25 would need to be repaid to reschedule the appointment. For example, to avoid losing the $25, if your appointment is on Wednesday you would need to let us know by 5pm the previous Tuesday.

    If you need to cancel your appointment with more than 7 days’ notice and are request a refund for the booking fee, a $5 handling fee will be charged to cover processing fees.

    I understand and agree to the deposit policy as stated. I understand that if I have any questions about payments made or would like to request a refund of any credit on my account, I should contact SNVC directly for that request. Any individual or account who initiates a bank or credit card Charge Back request against any amount paid to SNVC, not just this deposit, will incur a $50 finance fee and will not be seen by SNVC for any services until the account is cleared all money owed with a cash payment. In such case, the account or individual will be served on a cash only basis after the due amount is cleared and the individual will not be eligible for discounts or financial assistance.

      I, as the person with authority to make decisions regarding the above listed pet’s care, hereby authorize the selected services for my pet. 

      • I certify that my animal is in good health and not on any medications other than those disclosed.
      • I understand the exam performed may be a brief exam to determine if the pet is healthy for vaccines and is not considered a complete physical.
      • I understand the inherent risks of failing to maintain current vaccinations and the possibility of vaccine failure. I waive all claims arising out of or connected with the performance of these services or due to such vaccine failure.
      • I understand that vaccination reactions can happen and include: facial swelling, hives, reddened skin, itching, excessive swelling/ scratching at the injection site, pain at injection site, excessive drooling, lethargy, decreased appetite, vomiting, diarrhea, anaphylactic shock and / or death. I understand that vaccine and medication reactions are idiosyncratic and cannot be predicted. If there is a vaccine reaction, I authorize SNVC to perform any procedures deemed necessary to treat my pet.
      • I understand that feline Feleuk and FIV testing is recommended for kittens and annually afterwards, especially prior to vaccination.
      • I understand the heartworm testing is always recommended prior to administration of any heartworm prevention. 
      • I understand my medications given may not be in a childproof container.
      • I understand that SNVC has the right to refuse service to any animal determined to be ill or at increased risk. Sick animals are at higher risk for complications, as well as pose a risk to other animals in the clinic.
      • I understand that if there are complications my pet may need to receive post-operative monitoring or care at an emergency clinic and I accept the financial responsibility for that care if deemed necessary by the veterinarian.
      • I understand that photographs may be taken during the day, and my pet may be present in those photos. I understand that the photographs taken at SNVC may be used for promotions and advertisements for SNVC. This includes but is not limited to Facebook, Twitter, SNVC website, advertisements, etc.
      • I understand that if I do not retrieve my pet at the agreed upon time, SNVC may exercise their right to turn the animal over to the nearest animal control center, humane society, or dispose of as deemed just and proper as allowed by the State of North Carolina under G.S. 90-187.7(a). Owners of pets left after closing at 5pm will be charged a 25.00 per half-hour late fee. Owners dropping Pets off past the intake time of 9:00am will be charged a 25.00 late drop off fee and may be turned away and asked to reschedule.
      • I understand that payment in full is required prior to services being rendered.I understand that all sales are final and no refunds are permitted for services or medications.
      • I agree to allow SNVC to run any Credit Card(s) on file to cover any remaining balance due. Payments may be processed after the day of surgery.
      • I understand that under no circumstances will the Spay Neuter Veterinary Clinic pay for services received at another veterinary clinic, whether the services are related to a treatment or procedure done at the Spay Neuter Veterinary Clinic or not.
      • I understand that if the Clinic has any questions regarding your pet or there is reason for concern or increased risk with treatment noticed prior or during the procedure, the clinic will attempt to reach the owner at the numbers left at check in. IF THE OWNER IS NOT REACHABLE BY 10AM  I UNDERSTAND THAT THE PROCEDURES MAY NOT BE DONE ON MY ANIMAL and I may need to reschedule my appointment
      • I will not hold the veterinarian, their employees, volunteers, family members or SNVC responsible for any issues arriving from the procedures performed, such as vaccine or medication adverse events.
      • I hereby release the Spay Neuter Veterinary Clinic of the Sandhills, all veterinarians, assistants, volunteers, directors, and employees from any and all claims arising out of or connected with the performance of this procedure or any adverse reactions from vaccinations. I agree that I have not and will not claim any right of compensation from any of the above. I as the owner/agent hereby agrees to hold SNVC unaccountable for any damages caused during the transportation of the animal, while entering or exiting the service location, or for any damages caused by any unforeseeable events including fire, vandalism, burglary, extreme weather, natural disasters or acts of God.
      • SNVC does not offer hospitalization, radiology or emergency care. If there are any questions or concerns directly related to the services provided, please call this office at (910)400-7682 and press 3.
