I understand that failure to produce a photo ID matching the name above may result in Butterfest's refusal to hand out my tickets.
I understand that Butterfest will not be held responsible for lost or stolen tickets after they've been provided to myself and/or my designated pickup person. I understand that Butterfest volunteers may contact me if they have questions regarding my submission of this form. I understand that no refunds will be issued under any circumstances, including failure to pick up my tickets.
