1) Athlete First Foundation will take a minimum of 4-6 weeks to process the application by all the athletes. Shortlisted athletes will be contacted for the next process once all the applications are considered.
एथलीट फर्स्ट फाउंडेशन को सभी एथलीटों द्वारा आवेदन को संसाधित करने में कम से कम 4-6 सप्ताह का समय लगेगा। एक बार सभी आवेदनों पर विचार करने के बाद शॉर्टलिस्ट किए गए एथलीटों को अगली प्रक्रिया के लिए संपर्क किया जाएगा।
2) I authorize that the data from my assessement tests can be used by Athlete First Foundation for research and case studies.
मैं अधिकृत करता हूं कि मेरे मूल्यांकन परीक्षणों के डेटा का उपयोग एथलीट फर्स्ट फाउंडेशन द्वारा अनुसंधान और केस स्टडी के लिए किया जा सकता है.
3) I have no objections, that the photos and images taken during the screening process may be shared on print & digital media by Athlete First Foundation for marketing purposes.
मुझे कोई आपत्ति नहीं है, कि स्क्रीनिंग प्रक्रिया के दौरान ली गई तस्वीरों और छवियों को मार्केटिंग उद्देश्यों के लिए एथलीट फर्स्ट फाउंडेशन द्वारा प्रिंट और डिजिटल मीडिया पर साझा किया जा सकता है।
4) The final selection of the athletes to the scholarship program shall be on the discretion of the Athlete First Foundation and I shall not hold the foundation answerable in case I do not get selected to the scholarship program.
छात्रवृत्ति कार्यक्रम के लिए एथलीटों का अंतिम चयन एथलीट फर्स्ट फाउंडेशन के विवेक पर होगा और अगर मैं छात्रवृत्ति कार्यक्रम में चयनित नहीं होता हूं तो मैं फाउंडेशन को जवाबदेह नहीं ठहराऊंगा.
5) I hereby certify that the information provided above is accurate and to the best of my knowledge. I also certify that there is no condition,cardiac or otherwise, which precludes me from participating in any assessment or physical activity carried out by Athlete First Foundation.
मैं एतद्द्वारा प्रमाणित करता हूं कि ऊपर दी गई जानकारी सही है और मेरी जानकारी के अनुसार है। मैं यह भी प्रमाणित करता हूं कि ऐसी कोई स्थिति नहीं है, हृदय संबंधी या अन्यथा, जो मुझे एथलीट फर्स्ट फाउंडेशन द्वारा किए गए किसी भी मूल्यांकन या शारीरिक गतिविधि में भाग लेने से रोकती है।
6) I understand and agree that Athlete First Foundation shall not be held liable for any physical injury or medical condition occurring out testing, training or treatment during provision of any service in case of any false, incomplete or incorrect information furnished by me in this form.
मैं समझता हूं और सहमत हूं कि इस फॉर्म में मेरे द्वारा दी गई किसी भी झूठी, अधूरी या गलत जानकारी के मामले में किसी भी सेवा के प्रावधान के दौरान परीक्षण, प्रशिक्षण या उपचार के दौरान होने वाली किसी भी शारीरिक चोट या चिकित्सा स्थिति के लिए एथलीट फर्स्ट फाउंडेशन को उत्तरदायी नहीं ठहराया जाएगा।
7) I shall extend my full cooperation during the age verification process carried out by the Athlete First Foundation.
मैं एथलीट फ़र्स्ट फ़ाउंडेशन द्वारा की गई आयु सत्यापन प्रक्रिया के दौरान अपना पूर्ण सहयोग दूंगा
