I, pet owner, do hereby entrust Spud's Staycation to care for my pet while boarding during their stay and agree to all of the terms listed below. If I am unavailable to pick up my pet(s) on the previously listed date at the pre-arranged time, I understand I will be charged for additional days of boarding.
Health Disclosure: I authorize Spud's Staycation Canine Retreat to do what they deem necessary for the health and well being of my pet and agree to pay for any and all expenses relating to same should veterinary care be needed during their stay. We will make every effort to contact you should an emergency arise. I am aware that my pet, despite being vaccinated, could still be susceptible to contracting airborne illnesses such as kennel cough. My pet will be examined for fleas at check-in and treated if necessary at a charge of $20.00. Initial below.

We require all pets to be current on the following vaccinations: Rabies, DHPP, Bordetella. Proof is required prior to dropping off your pet.

Feeding Instructions

Please bring an adequate supply of your pet's normal food in a sealed container such as individual baggies or a plastic tote. Boarding in a new place can be stressful, and a new food will increase the chance of vomiting and diarrhea during their stay.
I understand that if I have not provided food my pet will be fed GI friendly Purina EN during their stay for an additional $5/day as noted above in the add-on section.*
