  • 2025-2026SY ENROLLMENT APPLICATION

  • We look forward to welcoming your family to St. Thomas Aquinas Academy!

Please complete this form to apply for the new school year. The form will first ask for general contact information for the family, then information about each of the students you are enrolling, and the final page of the application includes payment options.  Once enrollment is complete, the planning, advisor support, and member site support services for the 2025-2026 school year will be available to your family from the date of enrollment (the date the application and enrollment fees are submitted) until June 30, 2026.

You can save this form at any time to resume later simply by clicking the 'save' button at the bottom of the screen and entering your email.  When you save the form, you will see that Jotform, the company that provides the secure online form service, will ask if you want to create an account.  You do not have to make an account with Jotform to save your application progress.  Instead, look for the link on the save screen that says 'Skip Create an Account' and you will be prompted to enter only your email address so you can return to your saved application in the future.

Note: We recommend completing this application using Google Chrome, Firefox, or Safari browsers; Microsoft Edge is not always reliable.

To start your application, use the dropdown to select the number of students you are enrolling for the 2025-2026 school year and click 'Next' to continue.

  • ENROLLMENT CONFIRMATION

  • This is the petition to enroll your students and a confirmation of the terms you initialed earlier on the application.

Please enroll the children I have listed on this application in St. Thomas Aquinas Academy for the 2025-2026 school year.

I understand that enrollment fees are for standard assessment, planning, and academic support services only.  Books and materials are purchased separately through my favorite or St. Thomas Aquinas Academy’s recommended suppliers.

I understand that fees are not prorated.  Should our schooling situation change, a refund of fees is available for up to sixty days after our enrollment acceptance date (which will be printed in our welcome/ assessment packet), excluding the non-refundable registration fee ($125.00 per family) and the cost of all services utilized prior to the date of withdrawal (services such as assessment correction, assessment evaluation, development of personalized curriculum guide, planning appointment, and support appointments as well as postage).

I understand that there are no refunds after the Course of Study Planning appointment.  Nor are there refunds for unused services that are part of the enrollment package.

I understand that enrollment is for one grade level only.  My advisor will help add or make changes to my student's course list throughout the school year, but when it is time to plan for the next grade level, it is important to renew my student's enrollment and begin the assessment/planning process for the new school year.

I understand that enrollment is accepted from January 2025 through November 2025, active enrollment expires on June 30, 2026, and that all advisor appointments must be scheduled before 6/30/2026.
