    Use this form to add the Transcript Option to your student's annual enrollment. Submit this only if you did not add the Transcript Option to your student's enrollment when you submitted the application for the school year. Current enrollment is required for the Transcript Option and is not available as a separate service. If you submit payment for this option and your student is not presently enrolled in St. Thomas Aquinas Academy, a refund less a 5% processing fee will be issued to your card.
