By volunteering with Stars of the Future Inc., I understand and agree to the following:

Voluntary Participation: I understand that my participation in the activities and programs of Stars of the Future Inc. is entirely voluntary. I may withdraw from volunteering at any time without any consequences.
Scope of Activities: I have been informed about the nature of the activities I will be involved in and understand the expectations and responsibilities associated with my volunteer role.
Assumption of Risk: I acknowledge that there may be risks associated with volunteering, including physical, emotional, and social risks. I willingly assume these risks and agree to follow all safety guidelines and instructions provided by Stars of the Future Inc.
Confidentiality: I agree to respect and maintain the confidentiality of all personal information and sensitive data I may come across during my volunteering activities.
Code of Conduct: I commit to adhering to the code of conduct and ethical standards set by Stars of the Future Inc., including treating all individuals with respect and dignity.

Photo and Video Release

I give permission to Stars of the Future Inc. to take and use photos and videos of me during volunteer activities. These may be used for promotional purposes like social media, the website, and brochures. If I don't want my image used, I will inform the event organizer or contact Stars of the Future Inc. to opt out.

CORI Check

Thank you for your interest in volunteering with Stars of the Future. As part of our commitment to creating a safe and supportive environment for all individuals we serve, we require all volunteers to undergo a Criminal Offender Record Information (CORI) check in accordance with Massachusetts law and our organizational policies.

This CORI check is a standard procedure and is necessary to ensure the safety and well-being of our community. Please be assured that all information obtained will be handled with the utmost confidentiality and used solely for determining eligibility to volunteer.
