Terms and Conditions
Deposits
A deposit is required for each appointment that you book. This is non-refundable so be sure that your appointment is accurate and that all your questions have been asked. Your deposit is deducted from your appointment. If multiple sessions are agreed upon, it will be deducted from your final appointment.
If you are more than 30 minutes late for an appointment or cancel without respectable communication, your deposit will be forfeited, and you will be expected to provide a new deposit to rebook your tattoo. If you wait more than 12 months between appointments, a new deposit will be required to book.
Photos and Videos
The tattoo procedure may involve documentation in the form of photos and/or videos of both the process and the result. This documentation may be used for marketing and representation of the tattooists work, in digital and/or physical mediums.
Commitment
If a tattoo project is left neglected for more than 12 months, you may be refused continued service towards the completion of the project. Since creative workflows evolve over time, consistent appointments are necessary to maintain cohesion within style and quality. Please be sure that you are committed to completing a project before booking an appointment, especially when discussing large-scale projects.
Medical Advisory
Infection:
Acknowledge that infection is always possible as a result of the tattoo procedure, particularly if you do not properly care for your tattoo. Aftercare instructions are provided on the tattooists website (GlassSkinTattoo.com) and are readily available in verbal detail upon request. Any touch-up work needed will be done at your own expense, if accepted by the tattooist.
Medications and Disease:
If you have diabetes, epilepsy, hepatitis, hemophilia, HIV-AIDS or any other communicable disease, heart condition or take medicine which thins the blood, you must advise your tattooist.
Allergic Reaction:
Acknowledge that it is not reasonably possible for the representatives and employees of a tattoo establishment to determine whether you might have an allergic reaction to the pigments or processes used for your tattoo. By signing this form, you accept the risk that such a reaction is possible.
Age of Consent
You confirm that you are at least 18 years of age.
Opportunity to Communicate
After submitting this form, you will have the opportunity to ask any questions you may have. Upon submitting a deposit payment, you are declaring that you've reviewed this agreement and that you have been given the full opportunity to ask any and all questions which you might have about the tattoo procedure and that all of your questions have been answered to your full satisfaction. You specifically acknowledge that you have been advised of the facts and matters stated in this form.
