Front Desk Checkpoint
Each month, a leader will complete this checkpoint form for team members who work at the Front Desk. Our front desk team members have the important responsibility of ensuring a positive patient care flow each day. The goal of this checkpoint is to celebrate all the areas of proficiency and identify opportunities for growth. Please observe long enough to witness at least 2 patient check ins each month, and complete the form below.

Please check off each task/attribute that is completed and/or done correctly. Each checked box is worth a point, and the goal is to earn a 80% score each month.
Did the front desk team member warmly greet and welcome patients as they arrived?
Did they scan in medical insurance cards and initial it in Eyecare?
Did they correctly update recall, plan, and ID on the demographic?
Is the showroom tidy/presentable?
Is the bathroom tidy/stocked?
Is the front desk team member utilizing blue note? (if observing at an office where this is n/a, check the box to give them the point)
Did the team member double check all insurances at the start of the day to catch any errors?
Did the team member look ahead at the schedule to double check for any scheduling conflicts?
Is the team member assisting with quick check outs (if you don't see any opportunity to do a quick check out during your observation, then check this if the team member expresses that they are comfortable doing them).
Did the team member have a clear understanding of which patients were in the showroom, who is waiting for what, and the overall flow?
