  • COVID 19 VACCINATION ADVISORY

    In view of the latest recommendation from the Expert Committee and Health Science Authority, the Ministry of Health advised that all vaccinated persons should avoid exercise or strenuous activity for a period of 14 days after receiving ANY DOSE of the mRNA COVID-19 vaccines. As such, STRIKE Academy would like to request all STRIKE bowlers to take a 14-day break from bowling training after receiving both the first, second and booster dose of the vaccine as a precautionary measure.

  • IMPORTANT INSTRUCTIONS

      DO A HEALTH CHECK before attending training. Fill up this from within ONE HOUR before training and click SUBMIT at the end.
      CHECK LANE ASSIGNMENT before training so bowlers know to go to their lanes directly when they arrive at the bowling centre.  Lane Assignment details are appended below.
     

    PAY QM/LANE FEE (only those on such arrangement) before training. Paynow details are provided after the Lane Assignment below.

    LATE PAYMENT CHARGE of $10 apply if lane fee is paid late after training has started.

    bowler or members of same household have:    

    Symptoms or Covid19 positive   

    1. Tested Covid19 Positive (ART/PCR past 7 days) or excused on Medical Certificate.
    2. If bowler is unwell or a close contact (past 5 days), please perform and obtain a negative ART before leaving home and coming for training.  

    Bowlers who have recovered from Covid19 infections are exempted for 270 days from date of recovery. 

    If unsure, please contact the STRIKE Admin before start of training/coming to bowling centre. Do ensure you get an acknowledgement if you are not coming.

      3. Alternatively, if you are only using your mobile device, you can

              a. Take a picture of the QR code using your device camera.

        b. When activating the Scan & Pay / QR Pay in your Payment App, select  

                              "IMAGE FROM YOUR DEVICE" 

       4.  Take a picture of the Successful PAYMENT NOTIFICATION with BOWLER's NAME and attached it here (protect you and for verification)

      I hereby give my approval for my child’s participation in any and all STRIKE training and activities.  I absolve and hold harmless STRIKE and all its respective coaches, agents, and representatives from any and all liability for injuries and ailments to said child arising out of traveling to, participating in, or returning from selected training/activity sessions.
