Liability Waiver &
Terms Agreement
I, the undersigned, fully understand and agree to the following:
When I am participating in yoga or any classes, workshops and/or private sessions offered by Studio 4:8 Yoga, Inc whether in-person, online live or pre-recorded., I recognize that any physical exertion may be strenuous and may cause injury. I am fully aware of the risks and hazards involved.
I understand it is my responsibility to consult with a physician prior to and regarding my participation in these classes.
I am capable of judging the limits of my physical abilities and I understand it is my responsibility to stay within my range of ability.
I recognize it is my responsibility to notify my teacher of any serious injury or illness before every yoga class (whether in-person, live online or pre-recorded), workshop or private session.
In consideration of being permitted to participate in the classes (whether in-person, live online or pre-recorded), workshops and/or private sessions, I agree to assume full responsibility for any risks, injuries, or damages, known or unknown, which I might incur as a result of participating in any program offered by Studio 4:8 Yoga, Inc.
I knowingly and voluntarily waive any claim I may have against Studio 4:8 Yoga, Inc., and the Studio 4:8 Yoga teachers, Staff and/or owner for injury or damages I may sustain as a result of participating in any classes (whether in-person, live online or pre-recorded), workshops, private sessions, or programs.
I have read the release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above.
STUDIO 4:8 YOGA TERMS & POLICIES
Auto-renewal
Studio 4:8 Yoga keeps a student’s credit card on file for auto-renewal.
Auto-renewal memberships remain in effect until a request that it be terminated.
The recurring payment will either be on the 1st or 15th of the month, depending on when the initial membership sign-up occurs. The student will be prorated the difference.
Membership changes, cancellations or holds
Studio 4:8 requires a minimum 30 days notice from the next renewal date for changes, cancellations or holds on auto-renew memberships. Students must email studio48yoga@gmail.com to cancel.
Membership holds apply in medical circumstances, for up to two months. This hold can ensure a student can keep their current membership rate when they come back. A $24 hold fee per month applies.
Memberships
Intro Unlimited 2 Week Membership: This membership is available to new students only once. Unlimited classes are available 2 weeks from the first attended class. This membership may not be used with discounts to workshops or training.
Student Discount: Must be full-time college student taking 12 or more credits.
Buddy Membership: If a student’s buddy cancels, the student will either need to find a replacement for the buddy to keep the reduced rate, or the student will be transitioned into a single auto renewal membership. *Please note, a student cannot add a current member to their buddy pass. It must be a new or former member. Thank you for understanding.
In the event of a cancellation, the student may forfeit their current rate when they decide to come back. Membership expires one month after the final payment.
Failed Payments/Past Due Memberships
A student will be notified if their account payment fails to authorize for any reason. If not addressed within 24 hours, a $5 late fee will apply. If a valid payment is not made within 3 days of the original payment date, a $5 late fee will apply.
If a membership is 5 days past due, the membership will be suspended and the student will not be allowed to participate in classes. Reinstatement requires payment of dues owed and a reinstatement fee of $10. Memberships will be canceled after being suspended for 15 days.
Weather Cancellation Policy
If LPS cancels, we cancel our classes. If LPS is not in session, we post cancellations to our Facebook group and registered students will be emailed.

Cancellation Policy: Cancellations for private sessions made 24 hours in advance will be rescheduled. No refunds or reschedules will be offered for cancellations made with less than 24 hours notice.
