Please complete all 3 Separate Forms before your child participates in Summer Night Sports. All 3 forms must be completed and signed by a parent or legal guardian.

This is Form 1.

In consideration of _________________________ (Name of Minor Child/Ward) being allowed to participate in any way in Summer Night Sports youth program activities, training sessions, games, events, and related activities, I, _________________________ (Name of Parent/Guardian) the undersigned parent or legal guardian, acknowledge, appreciate, and agree that the risk of injury to my child from the activities involved in this program is significant, including the potential for permanent disability and death. While particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.

I, for myself, my spouse, and my child, knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the program organizers, volunteers, or other participants. I assume full responsibility for my child's participation in Summer Night Sports.

I agree to comply with the stated and customary terms and conditions for participation. If I observe any unusual or significant concern regarding my child's readiness for participation, I will remove my child from participation and bring such concern to the attention of the nearest program official immediately.

I, for myself, my spouse, my child, and on behalf of our heirs, assigns, personal representatives, and next of kin, hereby release, indemnify, and hold harmless Summer Night Sports, its directors, organizers, coaches, volunteers, officials, agents, sponsors, facility owners, and other participants from any and all claims, liabilities, incidents, injuries, disability, death, or loss or damage to person or property associated with my child's participation in this program, whether arising from negligence or otherwise, to the fullest extent permitted by law.

If any provision of this release is held invalid, the remaining provisions shall continue in full force and effect.

I, the undersigned parent or legal guardian of the above-named child, grant permission for my child to be photographed and/or recorded during activities or events associated with the organization listed above.

I understand that these photographs or recordings may be used for purposes including, but not limited to:

• Promotional materials
• Social media posts
• Websites
• Newsletters
• Printed publications

I understand that:

• My child's name may / may not be used alongside the images.
• These images may be used without compensation.
• The images will remain the property of the organization/photographer.

I release and hold harmless the organization, its staff, and representatives from any claims arising from
the use of these images.

This release is intended to be as broad and inclusive as permitted by the laws of the State of Michigan
and shall be governed by Michigan law.
