You must not visit the site if you or anyone in your household is self-isolating or is displaying COVID-19 symptoms. Instead, you must book a test via the NHS using this link.

In order to use the site you must:

Only arrive for your allocated slot, admittance outside this time will not be permitted.

Adhere to COVID-19 social distancing measures on the site to protect other staff and yourself.

Follow signage and staff guidance when moving around on site.

Bring a face covering with you and wear it whilst visiting the test site.

We are located at: Hill House, Bishopsford Road, Carshalton, SM4 6BL.

Privacy:

Upon arrival, you will need to register via an NHS web form. This will require you to enter some of your personal details and this data will be processed by the NHS. If you have a smart phone or tablet, please ensure that you bring it with you to complete this form. If you are unable to bring a smart phone or tablet, you will be able to borrow a device at the test centre to complete the form.
