Liability Waiver Form - 24 Hour Karate

In consideration of being granted access to and using the facility of 24 Hour Karate for independent training, I, the undersigned, acknowledge, appreciate, and agree that:
- I understand and accept that independent training activities involve inherent risks, including but not limited to the risk of physical injury, property damage, and contact with other participants.
- I acknowledge that I am solely responsible for my well-being and safety during independent training sessions.
- I agree to comply with all safety guidelines, rules, and regulations set forth by 24 Hour Karate and its staff while using the facility.
- I am aware that 24 Hour Karate, its owner, instructors, and staff are not responsible for providing supervision or guidance during my independent training sessions.
- I willingly assume all risks associated with independent training and accept full responsibility for any injuries, losses, or damages that may occur during my use of the facility.
- I hereby release, discharge, and hold harmless 24 Hour Karate, its instructors, staff, and affiliated organizations from any and all liability, claims, demands, actions, or causes of action arising out of my independent training activities.
- I agree to indemnify and hold harmless 24 Hour Karate, its instructors, staff, and affiliated organizations from any loss, liability, damage, or costs they may incur as a result of my independent training at their facility.
- I understand that this waiver and release of liability will bind my heirs, executors, administrators, and assigns, as well as myself.
- The owner is allowed to cancel membership anytime.
- I understand that any instructor is allowed to only (2) students at the time.
Individuals acknowledge that the dojo is equipped with cameras, and all of their activities will be captured on video.
- I understand that refunds will not be issued.
- I understand that a one-month notice is required for any cancellations.
- I understand that 24 Hour Karate collects membership fees for the first and last month.
- I understand that the instructor fee is $69.99.
- I understand that a student (non-instructor) fee is $39.99
Your subscription is non-transferable and cannot be transferred to another person and NO one else has permission to use your subscription.
