Hourly Appointment - $100p/hr
Half day - starts at 12:00, 2:00, or 4:00 - 4hrs - $350
Full day - starts at 12:00 - 7-8hrs - $600
For hourly and half day appointments, what times are best?
How soon are you wanting to get in?
Do you have a set budget or a gift card? What is the amount?
Are you ok with your tattoo being photographed and posted in my portfolio for others to see? (Private area tattoos will be censored!)
I understand that this is just a request and not a guarantee that I will receive an appointment.
If your request is denied, are you ok with your form being transferred to a different artist in our shop? (Please understand some designs may not be something I am comfortable with doing and I may find a different artist to be a better option.)
I understand it may take up to two weeks to hear a response to my form.
I understand that this is my consultation and if an in person consultation is needed I will be contacted for consult scheduling.
I understand that if my form is approved, I must pay a deposit prior to my appointment in order to get on the schedule. (Deposits secure your spot and go towards the total cost of the tattoo and are non-refundable.)
I understand that there will be absolutely no drawing or designing of my tattoo prior to my appointment, until the deposit is paid (Sorry we don’t draw for free!!!)
I understand that on the day of my appointment I must...
Be on time (I will give you up to 15 min. past the scheduled time, after that the appointment is an automatic no show)
No shows lose their spot and deposit, and will need to reschedule and pay a new deposit fee.
If I need to reschedule I will contact my artist 24 hours or more before my appointment.
If I am going to be late I will get ahold of my artist before the scheduled time so I am not marked as a no show.
I will bring a form of identification to fill out paperwork. (Drivers license or passport)
I have reviewed my form and made sure all information is correct.
