  • Background Release Form(s)

    As part of the onboarding process, Skylight conducts background checks on any individual who owns 25% or more of the equity interests of the company. Please fill and sign a Background Release Form for each owner with an equity interest of 25% or more. Additionally, a Background Release Form must be filled and signed by an individual with a significant responsibility for managing or directing the company (i.e. CEO, CFO, COO, Managing Member, General Partner, President, Vice President, Treasurer, etc.), or any other individual who performs similar functions. If the individual with significant responsibility is also a beneficial owner with 25% or more interest in the company, that individual should only fill and sign one Background Release Form. If there are no owners with 25% or more ownership interest, only the significantly responsible individual need to fill and sign the Background Release Form.

  • I, as the responsible party, give Skylight Lending permission to, either perform on their own or via an contracted outside firm, a background check on all significantly responsible individuals and owners meeting the criteria outlined in the paragraph above.
